Sleep Program
Sleep disorders are associated with heart disease, stroke, cognitive disorders, depression, obesity, and lower life expectancy. Over seventy million people suffer from chronic, severe sleep disorders in the United States. These include difficulties sleeping at night, problems with excessive daytime sleepiness, or other medical problems that may occur or be exacerbated during sleep. Our program consists of three separate sleep disorders centers as well as the Behavioral Sleep Medicine program. Planning Your Visit
Visiting the doctor’s office can be nerve-wracking, time-consuming and even annoying. A little preparation can go a long way toward minimizing the problems and optimizing the value of the visit.
First, get all the information and material necessary for the appointment ready before the visit. Remember to bring it with you. Forgetting the paperwork doubles the annoyance by having to repeat it.
Next, complete your sleep questionnaires. These are time-consuming but very important in helping us provide the best possible care.
Here are some helpful hints to help you schedule your appointment, prepare for the visit and ensure you know what to expect every step of the way.
A consultation appointment is an office visit with one of our sleep professionals. During the evaluation, your doctor will review your medical history and perform a physical examination.
You will be asked to discuss your sleep complaints as well as any other related conditions. Your physician will work with you to develop a treatment plan and may recommend an overnight sleep study.
It can be helpful to make a list of questions and important issues you would like to discuss to ensure all of your questions are answered.
In order to ensure your visit goes smoothly, please:
- Verify your insurance coverage and bring your current insurance card
- Obtain contact information for either your referring physician or primary care physician
- Complete the Sleep Questionnaire before arriving clinic
What to bring to your initial visit consultation appointment:
There are a number of key items that you should bring with you to your consultation appointment, including:
- Any previous sleep study results (if available)
- Your PAP machine, mask, and any other equipment (if applicable)
- A list of your medications
- Your physician’s contact information (if not already provided)
- Your insurance card
- Your co-pay (if applicable)
